This form needs to be submitted at least six (6) weeks prior to the requested event date. Requests are subject to availability and appropriateness of structure (see approval structure). Once the form has been submitted, you will be contacted via e-mail, telephone and/or written notice to confirm the event date. Until notification the event request is considered tentative.
